Free Range Family Wellness primary care provider Annemarie Hofmeister, NP wants you to be happy and healthy. Sounds like an obvious thought from a primary care provider, right? But actually, doctors who operate under the traditional fee-for-service insurance model benefit most when you are sick and need to be seen by them.

When you make an office visit, doctors get money from your insurance company. When you don’t, they don’t. Ever wonder why you hardly see your provider at an office visit or spend half your visit waiting to see them? They are dictated by how many patients they need to see in a day, which minimizes the amount of time each patient is able to spend with the provider.

By using a DPC model, Annemarie is able to focus adequately on the needs of each individual while personalizing their care plan and interventions, and help each patient meet their specific goals. With wholesale pricing for labs and images, patients can take comfort in knowing they will not need to wait extended periods for prior authorizations or receive bills for services that insurance did not cover. While this model may seem forward thinking, it’s truly back to the basics of how healthcare used to be with patients in charge of their own health.

Under the cash-pay Direct Primary Care (DPC) model we use, without having to worry about reimbursement costs, billing codes, or getting treatments approved or authorized from insurance companies, Annemarie is able to devote 100% of her time and attention to the direct needs and personalized care of her patients. When you schedule an appointment for an hour, you see Annemarie for an hour.

A monthly membership paid directly to Free Range Family Wellness provides you with unlimited access to healthcare similar to your monthly membership to your gym or health club. This approach ends up saving most people financially over the cost of a PPO or HMO insurance plan, even with subscribing to a supplemental high-deductible plan for high-cost situations like emergencies or specialty care. DPC models are especially beneficial to those with cost sharing programs, such as Solidarity or Medishare, where coverage only applies for visits that cost more than $1,000.

Independent from the restrictions of insurance or third-party billing, Annemarie is able to meet each of her patient’s personal health goals while taking into consideration all the nuances that play into them: religion, diet/health practices and preferences, lifestyle, family situation, and more. Her goal for after every office visit or virtual interaction is for her patients to feel their concerns were completely heard and understood.

About Annemarie Hofmeister

In addition to being a nurse practitioner, Annemarie is a mom, a wife, and a daughter. She has been raised to have faith as both the center of her life and her moral compass, and she values the role this has in her relationships with patients. She surrounds herself with people that bring her joy and loves spending time with her family and friends. She has four sons and two lovable black labs.

Annemarie is never afraid or ashamed to relate to another parent on the stresses of parenting, the imperfections of children, and the chaos of daily life. She encourages everyone to take care of themselves, no matter what season of life they may be experiencing, and she is a firm believer in the theory that you cannot give love when you do not love yourself.

One of Annemarie’s favorite things about her job is getting to know her patients on a personal level and deepening her community connections.   • Acute issues that can be handled remotely include sinus infections, ear infections, UTI’s, rashes, sore throats, yeast infections, etc.
  • Related to Wellness, we can discuss and order labs and screenings that need to be ordered. If a physical exam is needed, we can arrange an office visit.
  • Strep, flu, and UTI dip tests can be purchased in the office for at-home use.
  • We recommend that you purchase a camera otoscope if your child has recurrent ear infections. Otherwise, we can schedule an office visit or home visit to confirm an ear infection. Be Bird ear camera and Smart Otoscope are examples of brands we recommend on Amazon.
  • For kids 17 years of age and under, we utilize the American Academy of Pediatrics (AAP) guidelines for age-appropriate screenings and milestone checks. Parents will be provided with Ages and Stages Questionnaires for children 60 months and younger at each well child check; Bright Futures self-screening questionnaires will be sent to patients/parents prior to appointments for kids ages 6 and up as things to think about to spur any potential questions. Standardized screenings for ADHD, anxiety, and depression will also be used when appropriate.
  • For adults, Annemarie follows the USPTF guidelines for screenings for adults. She will order screening labs, colonoscopies, mammograms, prostate labs for screening, etc., as age appropriate. Chronic health conditions can also be evaluated at annual wellness visits, with follow-up visits throughout the year as needed.
  • We are not currently performing pap testing in the office but will update members when the service becomes available. We have several local providers in the area we recommend if you are due for the screening at this time. 
  • We will complete any school paperwork or immunization forms for one year following a well-child check.  

Yes! We recommend using these payment types if you have them available with your insurance plans. All medications, supplements, and visit costs in our office are FSA/HSA reimbursable. We provide an itemized invoice for all transactions for your proof of purchase.

  • In-person visits can be scheduled at our office location. Following signups, all in-person appointments will be subject to appropriate membership billing.
  • Pending availability, home visits can be arranged.

Yes. We will always offer wholesale meds if we have them in stock. The choice is yours which you would prefer.

If you are a member, you will be billed transparent wholesale pricing upfront and be provided with an itemized invoice. If you would prefer for your labs to be billed to insurance, that will be billed through local laboratories (UC Health or LabCorp) and sent with your insurance information for coverage; following service, these providers will send you their own bill. We are not responsible for insurance refusal to cover or increased prices at these third-party groups.

  • Lab work can be billed to commercial insurance through LabCorp or UC Health.
  • Medications can be billed to commercial insurance through your local pharmacy. Free Range Family Wellness will have certain medications in stock and will offer wholesale pricing as a potentially cheaper option.
  • Imaging ordered through Free Range Family Wellness can be billed to commercial insurance. This includes MRI, CT, Ultrasound, X-ray and more. Free Range Family Wellness will offer wholesale imaging pricing as a cheaper option to most commercial insurance plans.
  • Medical referrals to other specialty physicians can be billed to commercial insurance.
  • Durable medical equipment recommended by Free Range Family Wellness for health issues can be billed to commercial insurance through the equipment provider.

Our practice is designed to benefit you when normal life and the unexpected happens. When rashes, falls, bumps, or illness occur, we have you covered. No more spending your vacation time tracking down an urgent care or hoping that you can get to one before they close on a weekend. Our goal is to make life easier through providing peace of mind and compassionate care for you and your family.

Additionally, a single visit to an ER or urgent care can end up costing you the equivalent to months (or even more than a year) in membership cost in some cases!

Yes. Through a local partnership, we can provide vaccinations in the office at cash-pay rates or billed to commercial insurance plans. If you or your family are in need of vaccinations during your appointment, please reach out before your appointment to ensure we have a proper supply in the office.

No. We have opted out of Medicare and Medicaid, and our services cannot be reimbursed by these plans.

We recommend downloading the Atlas.md app for medical records access. Any questions about labs can be directly addressed in the app, or a text message or email can be sent to your provider to discuss in more detail.

We are also happy to share PDF results via email after they have been reviewed by the provider.

Yes. If no recent wellness visit or establishment of care has been obtained, your child will need to be seen for a visit. We also have access to the Colorado immunization state registry, so obtaining shot records for paperwork can be done as well.

Yes. If no recent wellness visit or establishment of care has been obtained, your child will need to be seen for a visit. After a wellness visit or establishment of care visit, all paperwork will be filled out for members for one year without visit requirements.

  • All payment for services, medications, supplements, and testing can be added as a charge to your profile and taken with regular payments on the 1st or 15th of the month.
  • Payments for services provided to non-members will be taken care of at the time of the service by billing a credit card or debit card.

Advanced notice of any planned absences will be provided to all patients in the practice. Any unanticipated absences for illness or family emergency will be communicated to patients as timely as possible. During these times, our staff will still be available via email, text, or phone call.
